ASSESSMENT AND
CASE MANAGEMENT:
Senior Services
Associates provides comprehensive needs
assessment and service coordination to assist seniors in gaining
access to and receiving needed services, with efforts made to
mobilize and coordinate formal and informal sources of support
on behalf of the older person. If appropriate, staff will
evaluate needs to pertaining to laundry, shopping, meal
preparation, housekeeping, personal care, etc. The need for home
delivered meals and adult day care services available in Kane,
McHenry and Kendall counties can also be determined.
